文頭の to think that の意味
文頭に「To think that」が置かれる場合があります。
意味は「まさか～だとは（驚きです）」で、何かを知って驚いた時の表現です。
後ろには that節が続きますが、that は省略されることがあります。
To think she was married.
→彼女が結婚していたとは。
To think that it's already been 10 years since we met.
→出会ってからもう10年経つとは。
To think that he spent his entire savings on the car.
→まさか彼がその車に貯金を全額つぎ込むとは。
To think that +and で2つの文を繋げる
「To think that」の後に「and」で文が続く場合があります。
To think that he used to be so shy, and now he's a party animal.
→以前の彼はとてもシャイだったのに、まさか今やパリピだとは。

このように、「～なのに、まさか～だとは / ～だなんて」のニュアンスで使われます。

Valda は女王の名前です。
その女王に「まさか娘がいたとは……」と驚いています。
「all along」は「ずっと」や「最初から」という意味があります。
そして、別の個所にも出てきます。

「何て愚かな！ まさか王がこの素晴らしい剣に興味がないとは驚きだ」
「simpleton」には「愚か者 / 馬鹿」という意味があります。
